Tesamorelin is a synthetic analogue of growth hormone-releasing hormone (GHRH) with a trans-3-hexenoic acid modification at the N-terminus for enhanced metabolic stability. It stimulates the anterior pituitary to secrete growth hormone in a physiological, pulsatile pattern, leading to downstream IGF-1 elevation and preferential visceral adipose tissue reduction. Tesamorelin (Egrifta) received FDA approval for HIV-associated lipodystrophy. Key studies by Falutz et al. demonstrated significant visceral adipose tissue reduction in clinical trials.
